Each All Star team is required to have a complete book/binder containing many documents that have to be completed accurately and prior to the Affidavit meeting.
Please provide the below documents to your team manager.

- Original Birth Certificate for each player
**If parent does not have original they can go to any courthouse in TX to obtain a copy.
 **8U  & 9U can provide a copy instead of original

- Copy of manager & coaches Driver's License
-Copy of certificate of completion of the Diamond Leaders Training   https://www.littleleague.org/diamondleader/

 - 2024 as school-enrollment-form.pdf    **ALL PLAYERS NEED THIS FORM!!
(date/signature from current date, home school students only submit 2 forms of residency, ex. Mortgage, electric bill, water bill, etc.) Collecting all forms and taking them all to the school may be the easiest.

- Volunteer Application  ***Managers and coaches only

Your binder should be in this  layout using dividers for each player and page protectors for each form front and back and for each player :
