Each All Star team is required to have a complete book containing many documents that have to be completed accurately and prior to the Affidavit meeting.
Please provide the below documents to your team manager.

- Original Birth Certificate for each player 
   **9U can provide a copy instead of original

- Copy of manager & coaches Driver's License

 -  school enrollment form.pdf
(date/signature from current date, home school students only submit 2 forms of residency, ex. Mortgage, electric bill, water bill, etc.) Collecting all forms and taking them all to the school may be the easiest.

- Volunteer Application  ***Managers and coaches only 

- Tournament Player Verification

-medical release form.pdf
(should be dated any day in November 2021)

-Parent code of conduct
(must be completed for each player)

- Report card
(must display schools name and players name, screenshot is acceptable)
